EHHD Cardiovascular Health Promotion Awards

Purpose

To recognize exemplary programs aimed at improving the cardiovascular health of members in schools, workplaces, and communities in the Eastern Highlands Health District. Exemplary programs address one or more of the risk factors for cardiovascular disease in an innovative or highly effective way. Risk factors include: physical inactivity; diets that do not follow the Dietary Guidelines for Americans: obesity: tobacco use: and high blood pressure.

Guidelines for entry

  1. Eligible schools, workplaces, or community groups must be in the geographic area served by the EHHD.
  2. There are three distinct categories for recognition: schools (k through 12), workplaces, and community (through town offices or community organizations).
  3. Only one application per group/organization will be accepted. In the event there are multiple applications from the same group/organization, the first one received will be the only one considered and reviewed.
  4. Only complete applications will be considered. A complete application consists of:
    1. Application form (available on-line at www.ehhd.org, at the EHHD main office in Mansfield, or mailed to you upon request)
    2. A typed description of your program of 500 words or less to include:
      1. The purpose of the program
      2. The target audience of the program
      3. How long this program has been in existence and any results or the impact of the program on your group
      4. What makes your program special (The people involved? The people you serve? The results you have accomplished? The way you have approached the risk factor?)
    3. No more than 4 supporting documents (pictures, letters of support, graphics of results, etc.)
    4. The specific areas of evaluation are:
      1. Innovation
      2. Effective approach in addressing one of the risk factors for cardiovascular disease
      3. Participation
      4. Results were attained and measurable (goals were met)
      5. Reproducible program (can be used as a model for others)
  5. Deadline for submitting applications is October 30. Applications can be hand delivered or mailed to: Health Education Program Coordinator, EHHD, 4 South Eagleville Rd., Mansfield, CT 06268

An Application Review Panel will evaluate all submissions to determine winners in each category. Awards ceremony will be in early December.
